WISEWOMAN

The WISEWOMAN program aims to reduce cardiovascular disease (CVD) risk factors in under or uninsured women 40 - 64 years of age through healthy lifestyle changes. 

How it works:

WISEWOMAN participants receive an assessment of blood pressure, total and HDL cholesterol, glucose, body mass index, and personal and family health history.  Participants also receive appropriate medical referrals based on the results of their screenings.


Participants who are ready to make healthy lifestyle behavior changes are offered lifestyle counseling to assist them.  The focused areas of change include nutrition, physical activity, and smoking cessation.  WISEWOMAN lifestyle counselors provide face-to-face and telephone contacts to support the participant in making progress toward her identified goals.  The lifestyle counselors also make referral to low/no-cost community resources, as needed.
